feat(cli,mcp): pad chat CLI and pad_chat MCP tool (TASK-4042) (#2295)

* feat(cli): add global pad chat commands (TASK-4042)

Add conversation targeting, inbox and message commands with caller-owned idempotency and precise read markers. Preserve API responses for JSON output and use cwd workspace links for reference context.

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(cli): add chat examples and isolate command tests (TASK-4042)

Satisfy the CLI help-tree example contract and restore package-level flags after command regression tests.

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* feat(mcp): expose global pad_chat on both transports (TASK-4042)

Add six chat actions with shared target parsing, uniform authenticated dispatch, precise read markers and structured unnamed-agent errors. Exclude agent chat from ChatGPT and pin the direction guard. Bump the surface to 0.102, update documentation and goldens, and shorten duplicated prompt prose within the fixed budgets.

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* test(mcp): label global chat scope judgments (TASK-4042)

Mark the membership-based global-chat exemption reasons explicitly, as required by the allowlist coverage guard.

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(chat): preserve unread gaps and successful outcomes (TASK-4042, seat r0)

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(chat): restore agent rules within prompt budgets (TASK-4042, seat r0)

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(chat): keep scope fixture contiguous with read marker (TASK-4042, seat r0)

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(chat): guard read boundaries and direction inputs (TASK-4042, r1)

Claude-Session: https://claude.ai/code/session_01KYZ6vrKNro953q9kfdSGFd

* fix(chat): validate MCP bodies and report mark outcomes (TASK-4042, r1)

## CLI
Global chat targets are `C<n>`, `<n>`, or `@<name>` for your DM with an agent.
All chat commands support `--format json`.
```bash
pad chat list
pad chat inbox [--agent NAME]
pad chat show <target> [--since N] [--limit N] [--no-mark]
pad chat send <target> <body|-> [--reply-to N] [--op-key K] [--context-workspace SLUG]
pad chat delete <target> <seq>
pad chat read <target> --to N
```
Show marks only the messages returned by the page read (unless `--no-mark`).
Replying marks read only to `--reply-to`; plain sends mark nothing. Send
ensures an unknown `@name` DM; other commands return not found. Body `-`
reads stdin. Reference context defaults to the cwd's `.pad.toml` workspace
link when present. Chat itself is global and needs no workspace.
Items are referenced by **issue ID** (e.g. `TASK-5`, `BUG-8`) wherever a `<ref>` argument appears.
Slugs also work but issue IDs are preferred.

Same placement rules as create. Field updates MERGE server-side: only
the keys you set change. Pass expected_seq (the seq you last read, on every item and list summary) to fail with code=update_conflict
if the item changed since; the details carry actual_seq. PREFER IT over
expected_updated_at, which cannot tell two writes inside one second
apart.
A 'content' write REPLACES the whole body, so send expected_seq with
it: read (get), edit that body, write it back with the seq you read.
On update_conflict with details.content_write, do NOT resend your
body (it would undo the change you missed): re-read, apply your edit
to the CURRENT content, and send that with the new seq. To overwrite
on purpose, send replace=true instead of a seq. A content write with
neither is flagged warnings.unguarded_content_write, and a later
release refuses it.
Writing 'content' while a browser tab has the item open goes to that
live document: the response echoes what you SENT with
warnings.content_outcome="applied_pending_flush" (it describes the
write, not the row), and a read before a tab saves still returns the
PREVIOUS content. That is lag, not a failed write: re-read later (get, or list with full=true; a default list carries no content), never
re-send. Nothing guarantees that save, so do not wait on it in a loop. The editor may normalise
the stored markdown, so compare on meaning, not bytes.
delete — Archive an item.
Required: ref.
restore — Un-archive (restore) a soft-deleted item by ref.
Required: ref.
get — Read an item.
Required: ref. Optional: lines ("40-80": numbered body lines plus
seq, for quoting a patch's anchors).
patch — Replace exact text in an item's body without sending all of it.
Required: ref, and edits ([{old, new, line_hint?}]) or undo (a patch_id
from an earlier patch answer, to invert it). Optional: expected_seq.
Each old must occur EXACTLY ONCE in the current body (quote more
context, or pass line_hint to pick among exact matches); edits apply
in order, all or nothing. The answer is a unified diff of what changed:
read it. Anchors guarantee position, not meaning: if the text around
your anchor was rewritten, the edit still lands. A moved expected_seq
does not refuse: the patch applies and returns rebased=true with the
intervening writes (complete=false when some left no record). On an
item open in an editor, the editor applies it to its live document,
merging with the typing there, and the answer carries
content_state="applied_pending_flush". Refusals write nothing:
patch_anchor_missing / patch_anchor_ambiguous (re-read, quote current
text), patch_unstable_roundtrip (the open editor would also change
content you did not edit: send a whole-body update instead) and
patch_room_open (an older editor that cannot apply patches holds it).
list — List items, optionally filtered.
Optional: collection, status, priority, parent, unparented, role, assign, all, limit, full.
parent and unparented are mutually exclusive. Results are
summary-shaped (no content bodies) unless full=true.
move — Move an item to a different collection.
Required: ref, target_collection.
link — Create a relationship between two items.
Required: ref, target, link_type.
field, fields, expected_seq, expected_updated_at, overwrite_pending_edits, replace.
Same placement as create. Pass expected_seq (last read seq, on every item/list summary):
a stale value returns update_conflict with details.actual_seq. PREFER IT over
expected_updated_at, which cannot distinguish writes within one second.
content REPLACES the whole body: get, edit, send with the seq you read.
On update_conflict with details.content_write, NEVER resend your stale body:
re-read, apply your edit to CURRENT content, send with the new seq.
replace=true deliberately overwrites instead of checking seq. Without either,
warnings.unguarded_content_write is returned; a later release refuses it.

delete — Archive. Required: ref.
restore — Restore an archived item. Required: ref.
get — Read. Required: ref. Optional: lines ("40-80": numbered body lines + seq for anchors).
patch — Exact body edits.
Required: ref, edits ([{old, new, line_hint?}]) OR undo (a patch_id from an earlier patch answer, to invert it).
Optional: expected_seq.
Each old must occur EXACTLY ONCE; quote more context or use line_hint to choose
among exact matches. Edits apply in order, all or nothing.
Read the unified diff: anchors guarantee position, not meaning; if the text around
your anchor was rewritten, the edit still lands.
A moved expected_seq does not refuse: the patch applies and returns rebased=true

the answer carries content_state="applied_pending_flush".
Refusals write nothing: patch_anchor_missing / patch_anchor_ambiguous (re-read,
quote current text); patch_unstable_roundtrip (editor would change unrelated
content: send a whole-body update); patch_room_open (an older editor cannot apply patches).
list — List/filter. Optional: collection, status, priority, parent, unparented,
role, assign, all, limit, full. parent/unparented are mutually exclusive.
Summary-shaped, no content bodies unless full=true.
move — Change collection. Required: ref, target_collection.
link — Add relationship. Required: ref, target, link_type.
link_type: blocks | blocked-by | supersedes | implements | split-from.
unlink — Remove a relationship.
Required: ref, target, link_type.
deps — Show all dependencies (incoming + outgoing) for an item.
Required: ref.
context — One brief before starting work: the item, parents
with acceptance criteria, matching conventions, deps, links.
Required: ref. Optional: intent, role, budget.
link-pr — Link a pull request to an item, from metadata you
supply; nothing is fetched from GitHub.
Required: ref, pr_number, pr_url. Optional: pr_title,
pr_state, pr_branch, pr_repo. Linking again replaces the
link. Answers with the item.
unlink-pr — Remove an item's PR link. Required: ref.
remind — Arm a one-shot reminder that fires at a specific instant.
Required: ref, remind_at (RFC3339 INSTANT — a bare date is
refused, since it names a 24-hour span rather than a moment).
When it fires, the item appears in pad_project next/ready
carrying the reminder_id, until you acknowledge it. Use this
when you defer work: it is how you ask to be reminded.
ack-reminder — Acknowledge a fired reminder so it leaves next/ready.
Required: reminder_id (from the fired suggestion, or from
the response when you armed it — NOT the item ref, since an
item can carry several reminders).
Nothing else acknowledges one: completing the item does not, since a reminder may be armed to fire
after the work is done.
reminders — List an item's reminders (armed, fired, acknowledged).
Each row's "id" is what unremind takes as reminder_id (and
ack-reminder, once that reminder has fired).
Required: ref.
unremind — Disarm (delete) a reminder.
Required: reminder_id (from reminders, or from the response
when you armed it).
claim — Atomically claim an item for execution (lease with expiry).
Required: ref. Optional: holder, worker_id, ttl (Go duration,
default 15m, max 24h). Answers with your worker_id.
Exactly one concurrent claimer wins; the loser gets a 409
naming the live holder and expiry — log it and skip, don't
retry in a loop. Re-claim while you hold it to extend the
expiry (heartbeat). The lease expires on its own: a crashed
holder blocks nobody past the TTL, and there is no reaper
to wait for.
release — Release an item's execution lease when you finish.
Required: ref. Optional: holder, worker_id (releasing someone
else's live lease is refused).
Idempotent: releasing an absent or expired lease answers released:false,
no error, so cleanup needs no check first.
star — Star an item for quick access.
Required: ref.
unstar — Remove star.
Required: ref.
starred — List starred items.
Optional: all.
comment — Add a comment.
Required: ref, message.
Optional: reply_to (comment ID for threaded reply).
list-comments — List comments on an item. Each carries its id and
edited (true once its body has been changed).
Required: ref.
edit-comment — Replace a comment's body. Only its author may.
Required: ref, comment_id, message.
delete-comment — Delete a comment. Anyone who may edit the item may.
One that still has replies becomes a placeholder (deleted: true,
empty body) its replies stay under; it goes when its last reply does.
Required: ref, comment_id.
backlinks — List inbound [[...]] references to an item ("Mentioned in").
Required: ref.
unlink — Remove relationship. Required: ref, target, link_type.
deps — Incoming/outgoing dependencies. Required: ref.
context — One brief before starting work: item, parents with acceptance criteria,
matching conventions, deps, links. Required: ref. Optional: intent, role, budget.
link-pr — Link supplied GitHub metadata; nothing is fetched.
Required: ref, pr_number, pr_url. Optional: pr_title, pr_state, pr_branch, pr_repo.
Replaces any previous link; returns the item.
unlink-pr — Remove PR link. Required: ref.
remind — Arm a one-shot reminder. Required: ref, remind_at (RFC3339 INSTANT; bare dates refused).
When fired, the item appears in pad_project next/ready carrying the reminder_id,
until acknowledged. Use when deferring work.
ack-reminder — Acknowledge a fired reminder so it leaves next/ready.
Required: reminder_id (from the fired suggestion or arming response, NOT item ref).
Nothing else acknowledges one: completing the item does not; it may fire after work is done.
reminders — List armed/fired/acknowledged reminders. Required: ref.
Each row's id is reminder_id for unremind (and ack-reminder, once that reminder has fired).
unremind — Disarm/delete. Required: reminder_id (from reminders, or from the response when you armed it).
claim — Atomically claim an execution lease.
Required: ref. Optional: holder, worker_id, ttl (Go duration; default 15m, max 24h).
Returns your worker_id. One concurrent claimer wins; a loser's 409 names the live
holder and expiry: log it and skip, don't retry in a loop.
Re-claim while you hold it to extend the expiry (heartbeat).
Leases expire: crashed holders block nobody past TTL; no reaper to wait for.
release — Release when you finish. Required: ref. Optional: holder, worker_id.
Another holder's live lease is refused. Absent/expired leases return released:false,
no error; cleanup needs no precheck.
star — Star. Required: ref.
unstar — Unstar. Required: ref.
starred — List starred. Optional: all.
comment — Add comment. Required: ref, message. Optional: reply_to (parent comment ID).
list-comments — List with id and edited flag. Required: ref.
edit-comment — Replace body (author only). Required: ref, comment_id, message.
delete-comment — Delete (item editors may). Required: ref, comment_id.
Replies retain a deleted:true, empty-body placeholder until the last reply is deleted.
backlinks — Inbound [[...]] references. Required: ref.
Optional: limit (default 50, max 300), offset.
Returns same-workspace rows first, then cross-workspace
rows (each cross-ws row carries source_workspace_slug).
Use it to answer "what other items reference TASK-5?" without scanning
all content.
history — Read an item's version history (newest-first, read-only).
Required: ref. Optional: limit (default 50, max 300 —
the NEWEST N versions), full.
Returns a token-light summary per version (id, created_at, created_by, source, change_summary);
the resolved content body is omitted. Restoring a
version is a web-UI action, not exposed here.
bulk-update — Update status/priority across multiple items.
Required: refs (array of refs, e.g. ["TASK-5", "TASK-8"]),
AND at least one of status / priority.
note — Append an implementation note to an item.
Required: ref, summary.
Optional: details.
decide — Record a decision on an item.
Required: ref, decision.
Optional: rationale.
export — Export a playbook or convention as a portable artifact.
Required: ref (PLAYB-N / CONVE-N, or slug).
Returns the artifact TEXT (YAML frontmatter + Markdown
body) as the tool result — ready to hand to import in
another workspace. Only playbooks and conventions are
exportable; the server rejects any other item type.
Read-only / side-effect-free.
import — Import a portable artifact as a new DRAFT item.
Required: artifact (the full artifact text a prior
export produced).
Creates a playbook or convention draft (the server
gates by the artifact's collection) and returns
{ref, slug, warnings}. The item lands as a draft —
review and activate it afterward. Mutating but not
destructive (creates an item, like create).
Same-workspace rows first, then cross-workspace rows carrying source_workspace_slug.
history — Read-only newest-first history.
Required: ref. Optional: limit (NEWEST 50 by default, max 300), full.
Summaries: id, created_at, created_by, source, change_summary.
full includes resolved content; restoring is web-only.
bulk-update — Set status/priority on several items.
Required: refs (array of refs), at least one of status / priority.
note — Append implementation note. Required: ref, summary. Optional: details.
decide — Record decision. Required: ref, decision. Optional: rationale.
export — Portable playbook/convention artifact TEXT: YAML frontmatter +
Markdown body, ready to hand to import in another workspace.
Required: ref (PLAYB-N / CONVE-N or slug). Only playbooks/conventions;
the server rejects any other item type. Read-only, side-effect-free.
import — Create a playbook/convention DRAFT (the server gates by the artifact's collection).
Required: artifact (full text from a prior export). Returns {ref, slug, warnings};
review and activate afterward. Mutating, not destructive (creates an item).
ALWAYS prefer issue refs (TASK-5, IDEA-12) over slugs.
