docs: add deployment examples index and cross-link migration guides
Create docs/examples.md as the central entry point for all 5 turnkey docker-compose scenarios with a decision matrix, per-example summaries, and contextual migration guide links. Update quickstart.md to bridge from demo to real deployment. Consolidate README docs table (10 rows from 13). Fix Vault PKI "(planned)" in cert-manager guide.
