Paul Lorenz 9cb8373b9e Seed the router data model index gate from the applied raft index. Fixes #4439 ...

- records the database's persisted raft index on the FSM at init and exposes it as GetStartIndex
- surfaces it on the host controller as GetStartRaftIndex
- seeds the router data model's index provider from it rather than from the raft log tip, which is ahead of applied state while a node catches up and makes the model reject every changeset the FSM then applies
- tests that a restarted FSM reports the index the database holds
