Khalid Abdi 2454bb4c2b backend: version + network endpoints, LAN-friendly CORS/origins
Add public GET /api/version (current version + GitHub-release update check,
cached, fail-soft) and GET /api/network (detected LAN addresses). Accept
localhost/private-LAN origins in CORS and Better Auth trusted origins via a
shared isAllowedOrigin helper, so staff can reach the app over the clinic
network. The seed-demo.ts fake test data is removed.

Khalid Abdi a68b7f2573 fix(backend): make tunnel relay reachable before baking it into QR
Two changes so off-network wallet scanning actually works:

- Force the cloudflared edge connection over http2 (TCP/443) instead of
  QUIC (UDP/7844) in both the Docker tunnel and the dev-tunnel script.
  QUIC is blocked on many networks/Docker setups, which left the tunnel
  stuck "Failed to dial a quic connection" and the QR pointing at a dead
  URL — the root cause of "must be on the same network" failures.
- Gate the discovered quick-tunnel URL on real end-to-end reachability:
  poll the tunnel's own /health until it answers (Cloudflare 1033 clears
  in ~30s) before publishing it, and have /pair await that discovery so
  the QR never carries a not-yet-live URL.

Khalid Abdi 014b4ddf8f feat(backend): one-command Dockerized cloudflared tunnel
Add `npm run docker:tunnel` (docker-compose.tunnel.yml overlay): a
cloudflared sidecar opens a public quick tunnel to the backend, and the
backend auto-discovers its https://…trycloudflare.com URL from cloudflared's
metrics endpoint (services/relay-url.ts) and bakes it into the wallet-import
QR — so a phone on any network can scan, with no install/account/PUBLIC_RELAY_URL.

PUBLIC_RELAY_URL still wins when set. Verified end-to-end: the backend logs
the discovered tunnel URL on startup.

Khalid Abdi 2d47abcc42 feat: patient wallet — real Signing, encrypted share relay, import-from-app
Backend
- clinic Ed25519 signing key (services/signing.ts, routes/signing.ts,
  clinic_signing_keys table) — Settings → Signing is now real
- @noble wallet-crypto (lib/wallet-crypto.ts): ed25519 identity, base58check
  wallet numbers, sealed-box (x25519 + xchacha20poly1305)
- /wallet Socket.io relay namespace (challenge-signed device auth) forwarding
  only ciphertext; emitToWallet helper
- import-from-app flow (routes/patients-wallet.ts, services/wallet-share.ts):
  request-share → patient approval → decrypt + verify → review draft → commit
- temporary shares: patients.share_expires_at + 5-min auto-delete sweep; revoke

Frontend
- SigningPanel wired to live key/fingerprint/rotate + shared-records list
- "Import from a patient app" dialog (lib/signing.ts, import-from-wallet-dialog)
  reusing the draft-review path; temporary badge on the patient list

Khalid Abdi b74d5d2d05 feat: username reset, system-message UX, meetings polish, sidebar identity
- auth: forgot-password now has Email/Username tabs; new public
  /api/auth-helpers/reset-by-username resolves a username and hands off to the
  existing reset flow (real email or admin-notify fallback)
- messages: conversations expose isSystem; System notices are read-only (no
  composer, no call button) and styled distinctly (shield icon + badge)
- meetings: compact, larger control bar; self tile shows real initials and an
  avatar (not black) when the camera is off; delete-room UI with confirm
- sidebar: username-only accounts show @username instead of a synthetic email

Khalid Abdi 516de6ad60 portal: public Patient Portal kiosk (self-service booking + results)
New chrome-less (portal) route group with /portal/[clinic], a card-style choice
of "Book an appointment" vs "View my results", and step flows wired to a new
public backend router (src/routes/portal.ts):
- GET  /api/portal/:clinic            -> clinic name
- POST /api/portal/:clinic/appointments  (verifies name+file#, books a confirmed
  appointment that appears on the doctor's Appointments page)
- GET  /api/portal/:clinic/results    (minimal, safe status — no clinical values)

Excluded /portal from the auth proxy redirect so the kiosk loads without a
session. PHI exposure is intentionally minimal (see docs).

Khalid Abdi c88b674196 feat(meetings): Discord-style staff voice/video calls under Messages
Backend: meeting_rooms table + /api/meetings (list/create/delete, org-scoped),
and WebRTC mesh signaling over the existing authed Socket.io (call:join with a
≤4 cap and org authorization, call:signal relay, peer-joined/left, disconnect
cleanup). Migration 0024.

Frontend: Messages nav is now expandable (Inbox + Meetings); new
/messages/meetings page with a room list and a Discord-style call UI — a
useWebRtcMesh hook (camera/mic, screen share via replaceTrack, ≤4 mesh) and a
tile grid with a mic/camera/screen/leave control bar. New lib/meetings + i18n.

Khalid Abdi 43eaccb97e feat: HL7/FHIR, e-prescribing & insurance claims integrations
Real, standards-compliant integration clients that the clinic points at
its own (sandbox or production) endpoints — no mock data.

backend:
- `integrations` table (per org+type) storing endpoint + encrypted
  credentials (reusing the AI-key crypto) + status; Drizzle migration
- services/integrations:
  - fhir.ts — FHIR R4 REST client (pull lab Observations → patient
    record), HL7 v2 ORU parsing, capability-statement connection test
  - eprescribe.ts — NCPDP SCRIPT NewRx message build + transmit
  - claims.ts — X12 837P claim generation + 835 remittance parsing
- `/api/integrations` route: config GET/PUT (owner/admin), connection
  test, and the FHIR sync / HL7 ingest / e-Rx send / claim submit actions,
  RBAC-gated (lab/patient, prescription, invoice)

frontend:
- lib/integrations.ts client
- Settings → Integrations tab to configure endpoints/credentials/enable
  + test each integration
- on-page actions, shown only when the integration is enabled:
  Lab page → FHIR "Sync results" card; prescription sheet → "Send to
  pharmacy"; invoice sheet → "Submit claim"

Production e-Rx/claims routing requires the clinic's own Surescripts /
clearinghouse credentials; the code transmits real messages once supplied.

Khalid Abdi b1abb29108 feat: patient & lab file attachments (real backend storage)
Add a real file-storage layer to the backend and wire upload UI into the
frontend.

backend:
- new `attachments` table (org-scoped, links to a patient file number and
  optionally a lab result) + Drizzle migration
- `/api/attachments` route: upload (multer → disk under UPLOAD_DIR),
  list, stream/download, delete; gated by patient:write OR lab:write via
  a new requireAnyPermission helper so lab staff can attach analyses
- UPLOAD_DIR env (default ./uploads) + a persistent docker volume

frontend:
- lib/attachments.ts client (multipart upload, list, delete, preview URL)
- staged file picker in the patient Add/Edit dialog (uploaded after save)
  and the lab Add-result dialog (linked to the result)
- a Files section in the patient sheet that lists attachments and opens
  them in a preview dialog (images inline, others via download)

Khalid Abdi f0fe501d62 backend: prescription start/end dates + dispense ledger
Add optional startDate/endDate columns to prescriptions (schema,
validation, types, service) — when set, endDate drives expiry.

Add a new append-only `dispenses` resource (schema, types, validation,
service, route at /api/dispenses) recording who received which medication,
gated on the existing inventory RBAC statement. Migration 0020.

Khalid Abdi 15fc7fdf26 feat(messages): attach files and share appointments
- new message_attachments table + POST/GET /api/conversations/attachments
  (base64 upload, capped 10MB; authed clinic-scoped download)
- messages carry an attachments JSON column (file refs or appointment
  snapshots); realtime + REST send accept attachments and allow
  attachment-only messages; migration 0019
- composer "+" menu (Files / Appointments): file picker uploads and
  stages chips; appointment picker searches by patient and attaches a
  snapshot; both render in the thread (download chip / appointment card)
- raise express json limit to 15mb for base64 uploads

Khalid Abdi d096c4fe9d feat: invoices — patient billing with installments + PDF export
Backend (new `invoice` RBAC resource, granted to clinicians + reception):
- invoices table (line items + installments as JSONB), types, zod validation,
  service (CRUD + splitIntoInstallments + auto invoice numbers), org-scoped
  REST routes mounted at /api/invoices, activity logging (migration 0015)

Frontend:
- lib/invoices.ts API client + money/date helpers
- /invoices page: list with KPIs and search, create/edit dialog (searchable
  patient combobox, inline line-item editor, live total), detail sheet to split
  a bill into equal monthly installments, delete, and Download PDF
- dependency-free PDF via a print-styled window (browser "Save as PDF")
- sidebar "Invoices" entry under the Patients group; "Added by AI" badge honored

Khalid Abdi 3a7378e00d backend: AI chat agent with Veil PHI safeguard (providers, /chat, import)
Real LLM chat replacing the mock, backend-centric per the plan:

- Multi-provider API-key mode (OpenAI / Anthropic / Gemini via the AI SDK) plus
  local Ollama (OpenAI-compatible endpoint). Provider is derived from the
  picked model id; the matching stored key is used. New user_ai_settings table
  holds per-user config with provider API keys encrypted at rest (AES-256-GCM,
  src/lib/crypto.ts, keyed by AI_CREDENTIALS_KEY).
- POST /api/ai/config (get/put, secrets never returned), POST /api/ai/test
  (Ollama ping / key presence), POST /api/ai/import (approved migration commit,
  re-validated server-side, reuses the audited patient service).
- POST /api/chat: streamText agent with tools (getPatient, getPatientLabs,
  searchPatients, previewImport). Real record data streams to the clinician as
  custom data parts (cards) while the model sees only Veil-redacted results.
- Veil (src/services/ai/veil.ts): de-identifies patient identifiers to tokens
  before external calls, resolves tokens on tool args, and rehydrates the final
  answer. Bypassed for local Ollama. External mode runs non-streamed so the
  rehydrated text is correct. Every call is audited (provider + Veil level).
- Shared role-scoping helpers extracted to src/lib/role-scope.ts (reused by the
  patient routes and chat tools so visibility rules match).

Khalid Abdi 67fdf15de4 backend: org-scoped pharmacy inventory model
Add an `inventory` resource mirroring the prescriptions feature end-to-end:
Drizzle table (org-scoped, indexed), domain type, zod validation, service
(list/get/create/update/delete), and an RBAC-gated CRUD router mounted at
/api/inventory. Grant the new `inventory` statement to roles — pharmacy gets
read/write, full clinicians read/write/delete, reception/lab none — in both the
backend access control and (mirrored) the frontend. Record writes in the
activity log via a new `inventory` entity type. Includes the migration and an
idempotent demo seed script.

Khalid Abdi 6213da9477 feat: admin-provisioned staff, username login & role-based access
Replace the email-invitation flow with admin-provisioned staff accounts and
add role-based access that changes what each member sees.

Backend:
- Enable Better Auth `username` plugin (staff sign in by username); regenerate
  auth schema (+ username/displayUsername on user) and migration 0007.
- Add `doctor` and `reception` roles to the access-control RBAC. `reception` is
  scoped to scheduling + registration (no `prescription` statement).
- New `/api/staff` route: POST creates a user (auth.api.signUpEmail) and adds
  them to the active clinic (auth.api.addMember); GET lists members + usernames.
  Gated by requirePermission({ member: ["create"] }).
- Redact clinical PHI for the reception role in the patients service (read,
  create and update) so demographics-only is enforced server-side.

Frontend:
- usernameClient + Email|Username tabs on the login form.
- lib/roles.ts: useActiveRole + Better-Auth-permission-driven nav visibility,
  default landing, and a route guard (reception -> /appointments, blocked from
  clinical routes). Applied to the sidebar, command palette and auth guard.
- Care team page now provisions staff via a two-step Add-team-member dialog
  (details -> username/password) hitting /api/staff; removes the email-invite
  and pending-invitation UI. New members are contactable from Messages
  automatically (they become org members).
- Hide clinical sections of the patient form and the admin-only settings tabs
  for non-clinical/non-admin roles.

All permission management stays in Better Auth (per the better-auth skills now
referenced in backend/CLAUDE.md).

Khalid Abdi 730e07fcfc feat: real-time staff messaging over Socket.io
Add conversations/participants/messages tables, a participant-scoped REST
API (/api/conversations) and a Socket.io server (session-authenticated
handshake; per-user + per-conversation rooms) sharing the HTTP port. New
messages broadcast live and create per-recipient notifications. Also lands
the notifications table + service + routes (used by the message flow). The
Messages page is rewritten: live threads, unread state, and a compose
dialog to start a conversation with a clinic member.

Khalid Abdi 48378ebc5e feat: computed analytics endpoint, rework Analysis page
Add GET /api/analytics returning real aggregates over the clinic's
patients/appointments/prescriptions/tasks, and rebuild the Analysis page
to render them. Drops the fabricated revenue/profit cards — temetro has no
billing data source.

Khalid Abdi 75940313a4 feat: activity audit log written from all resource routes
Add the activity_log table, a best-effort recordActivity() service and a
GET /api/activity feed, and write entries on create/update/delete of
patients, notes, appointments, prescriptions and tasks. The Activity page
now shows the real audit trail (actor, action, patient context, time);
the fabricated signing hashes / approval badges are gone — that vision
stays deferred.

Khalid Abdi 7ef9da59bd feat: org-scoped prescriptions backend, wire prescriptions page

Khalid Abdi dec04ec506 feat: org-scoped appointments backend, wire appointments page
Add the appointments table, validation, service and CRUD routes
(/api/appointments, RBAC-gated) and the matching frontend data module.
The appointments page now loads and persists real data; KPIs are computed
from it and the schedule/calendar anchor to the real current date.

Khalid Abdi 7d6641ef59 Add notes API: clinic + author-scoped CRUD
New `note` table (src/db/schema/notes.ts) referencing organization + user,
with org+author scoping so a doctor only sees their own notes within the active
clinic. Adds:

- src/types/note.ts + src/lib/note-validation.ts (zod)
- src/services/notes.ts (CRUD, treats non-uuid ids as not-found)
- src/routes/notes.ts mounted at /api/notes, gated requireAuth → requireOrg
- schema barrel + generated migration drizzle/0001_*.sql (applied on startup
  by the runtime migrator)

Claude 9dabe2f5d2 Add TypeScript + Express + Postgres backend with Better Auth
Implements the first temetro backend: an Express 5 API on Postgres via
Drizzle ORM, with authentication and multi-tenant clinics powered by
Better Auth.

- Auth: email/password with required email verification, password reset,
  rate limiting, CSRF/trusted-origins, secure cookies, session audit hook.
- Organizations (clinics) with RBAC (owner/admin/member/viewer) and an
  extended `patient` permission set; member invitations by email.
- Org-scoped patient records mirroring the frontend Patient shape, with
  CRUD endpoints gated by permission (read/write/delete).
- Email helper logs links to the console when SMTP is unset (zero-setup
  local dev); Dockerfile + docker-compose (db + backend + frontend) with
  migrations applied on startup and a configurable Postgres host port.
