A clinician can push an updated record to a wallet-linked patient (permanent
share). The snapshot is signed with the clinic Ed25519 key and sealed to the
wallet's X25519 key — derived from its Ed25519 wallet number via the birational
map, verified byte-for-byte against the wallet's own derivation. Stored pending,
delivered over the /wallet relay live and on the wallet's next authenticated
connect (offline catch-up). The patient approves/denies in-app; the wallet signs
its decision, the backend verifies it, and the record is replaced only on
approval. Wallet pins the clinic key (TOFU) and warns on change.
Backend: walletRecordUpdates table + service, ed25519PubToX25519Hex helper,
POST /api/patients/wallet/push, GET .../link/:fileNumber|updates|updates/:id,
wallet:update-request / wallet:update-response relay events.
Frontend: "Push to wallet" dialog with live status, wallet-link gating on the
patient sheet, "Sent updates" list under Settings → Signing, walletPush /
walletUpdatesList locale namespaces across all five languages. Bumps to v0.5.0.

Record a clinician↔patient visit (or paste a transcript) on the patient
sheet; the backend transcribes it (OpenAI Whisper / Gemini), de-identifies
the transcript + context through Veil, and drafts a structured SOAP note
the clinician reviews and edits before saving — the same write-approval
gate as the chat agent.
Backend: POST /api/scribe/{transcribe,draft,save} (routes/scribe.ts,
services/ai/transcribe.ts), veil.redactText() free-text redactor,
appendEncounter service, audio MIME types on attachments. Gated by
patient:write + the clinic AI policy (reception/disabled-AI excluded).
Frontend: ScribeDialog + lib/scribe.ts, "Record visit" on the patient
detail, gated by clinical access + AI availability. New `scribe` locale
namespace across all five languages. Bumps to v0.4.0.

The record graph was a cramped React Flow box wedged inside a sheet
section. Redesign it Obsidian-style — round nodes with the label
underneath, a soft glow, and a hover-focus that highlights a node plus
its neighbours while dimming the rest.
Move it out of the inline section: the sheet now shows an "Open graph"
button (closes the sheet, opens the graph in a large dialog) and a
clickable list of the patient's records (problems + visits); clicking a
record opens a detail dialog.

Stop blocking AI imports/proposals on missing non-critical fields. Records
the chat agent drafts now save with safe placeholders, auto-generated file
numbers, and a source="ai" marker that surfaces an "Added by AI" badge so a
clinician can review/edit them later.
Backend:
- add `source` (manual|ai) column to patients/appointments/prescriptions
(migration 0014) + canonical types, services, validation schemas
- relax patient/appointment validation: empty file number allowed, demographic
+ type/provider/initials fall back to placeholders (initials derived from name)
- patients.generateFileNumber() auto-assigns an MRN when one is missing
- proposeAppointment accepts a name when no file number resolves; AI commits +
/api/ai/import stamp source="ai"
Frontend:
- `source` on Appointment/Patient/Prescription types; AI commits send source="ai"
- reusable <AiBadge> shown on the Patients table/detail and prescriptions list

- Messages: search the inbox and the compose member picker.
- Care Team: clickable member rows open an employee dialog showing role +
permissions, with change-role (updateMemberRole) and remove.
- Patients: Primary Care is now a provider dropdown (defaults to self for a
doctor); add a Transfer action + dialog wired to the transfer API.
- Activity: entries are clickable, opening a detail dialog.
- Analytics: Section takes a columns prop so each row fills evenly (no orphan
card in Appointments).
- Add lib/staff.ts (listProviders), transferPatient client, rolePermissionSummary
helper, and i18n keys for all new strings.

Finish the i18n pass: settings panels (profile/care-team/signing), the
chat heading + input, the patient cards / detail / create-edit form, and
the notes page + rich-text editor are all keyed in en/translation.json.
All 526 static t() keys resolve. Document the new backend resources +
Socket.io realtime (backend README/CLAUDE) and the i18n coverage
(frontend CLAUDE).

Round-2 fixes to the 6 features:
- Logo: enlarge the sidebar mark, drop the inline wordmark, and show
"temetro" via a hover tooltip; bigger logo on the auth screens.
- Sidebar footer: wrap the quick-nav, clinic switcher, and user menu in a
single bordered block so it reads as one footer instead of three cards.
- Sidebar nav: highlight the active page (usePathname + data-[active]), and
add an Appointments & Schedule sub-page under Patients that auto-expands
for the current section. New mock /appointments page; reachable via ⌘K.
- Notes: redesign to a two-pane list + editor with an Empty state on the
right when nothing is selected; fix the editor so text starts top-left
(div instead of a centering <button>); compact the toolbar; add
.note-content typography in globals.css so headings/lists/etc. actually
render (the app has no typography plugin).
- Patients: new PatientDetail laid out to fit the side Sheet (stacked
full-width sections, no nested click-to-expand dialogs); keep Edit.

Several navigation/UX additions:
- Command palette (⌘K): components/command-palette.tsx wraps the app shell
(mounted in app/(app)/layout.tsx) with a controlled COSS CommandDialog listing
the nav pages; a "Quick nav" Kbd button in the sidebar footer also opens it.
Installed @coss/kbd. Extracted the nav list into lib/nav.ts so the sidebar and
palette share one source of truth.
- Clinic switcher moved from the sidebar body into the footer; its menu now
opens a read-only "Clinic info" dialog and a "Create clinic" dialog instead of
routing to /onboarding. Shared CreateClinicForm (components/clinic/) is reused
by onboarding.
- Patients: clicking a row opens a right-side Sheet with the full record
(components/patients/patient-detail-sheet.tsx) instead of navigating to the
chat; PatientResult gained a vertical "column" layout for the Sheet.
- New Analysis page (app/(app)/analysis/) — a mock dashboard (revenue/profit,
patient volume, appointments, operations) reusing Sparkline + Card + Badge.
- Logo: set metadata.icons so the new mark appears as the browser-tab favicon.
