joshsiv-mulga 05f390e618 refactor: move spx command tree to operator/cli ...

Moving the command tree out of cmd/ brings it under errcheck and gosec.
Flag and config wiring now fails loudly instead of silently dropping a
binding, listing commands fail when they cannot write their table, and
the formation server's unauthenticated TLS is annotated as a known gap.
