fix(scanner): discover sub-quorum heal candidates (#6384)

* fix(scanner): preserve unversioned heal retries

* fix(scanner): bound orphan heal discovery fallback

* fix(filemeta): fence unsafe heal key components

* fix(scanner): preserve exact overflow heal versions
