* fix(config): append env host rules to RENOVATE_CONFIG ones

List options set through their own env variable, like
`RENOVATE_HOST_RULES`, replaced the list from `RENOVATE_CONFIG`. Append
them instead when the option is mergeable (`hostRules`, `customManagers`),
the same way file and env config are combined.

* test(config): cover customManagers from env without RENOVATE_CONFIG

Drop the `mergeable` check from the list-of-objects branch: only
`hostRules` and `customManagers` reach it, and both are mergeable, so the
check could never be false. Add a test for `RENOVATE_CUSTOM_MANAGERS`
without a list in `RENOVATE_CONFIG`.
