The new report-isolation fixture exposed existing reference-definition fallthrough: a trailing non-rendered URL could turn None or an empty secondary-topic field into a decomposition task. Omit definition tokens from the visible projection, preserve their link resolution context, and retain literal examples as evidence. Strengthen concurrent report and revision isolation controls.

Contract-Neutral: This restores the existing visible-declaration boundary for issue metadata; no new classification, communication policy, permission or product runtime contract is introduced.
