rcourtman ee8a24e14a backend and governance: MCP contract, agent capabilities, API, and release-control
Manifest-backed MCP tools, prompts, and resources with surface affordance contracts; agent capability manifest and governance projection; API contract tests and capability route projection; operations-loop and intelligence-funnel telemetry; release-control subsystem documentation, registry, and tooling; licensing and configuration.

rcourtman faefe6edc8 Remove 198 unreachable Go functions
Dead-code sweep. Functions flagged unreachable by golang.org/x/tools/cmd/deadcode
and confirmed unused across pulse, pulse-enterprise, pulse-pro and pulse-mobile by
adversarial cross-repo verification. Cross-module reachability was checked
explicitly (only pkg/ exported symbols are importable by other modules; internal/
packages and _test.go files are not). go build, go vet and test-compile all pass.

rcourtman 0f4c831fab Reject expired trials in hosted subscription gate
isHostedSubscriptionValid was treating any tenant with a non-nil
TrialEndsAt as a valid bounded trial, never comparing the timestamp to
the current time. A tenant whose trial ended in the past but whose
stored subscription state was still "trial" (e.g. lease refresh failed
or never ran) retained full hosted Cloud access indefinitely.

The validity rule now requires both that a trial end timestamp exists
AND that time.Now() is before it. Refactored the switch to make the
trial branch read explicitly: nil evaluator -> deny, nil end -> deny,
past end -> deny, otherwise allow.

Added TestTenantMiddleware_HostedMode_ExpiredTrial_Blocked as the
regression test (mirrors the existing BoundedTrial_Allowed +
UnboundedTrial_Blocked pair with a past-dated trial end).

rcourtman 728c42e47b Bring action endpoints onto the agent surface with the agent-stable envelope
Closes the last known gap in the agent substrate. The three
action endpoints (POST /api/actions/plan, /api/actions/{id}/decision,
/api/actions/{id}/execute) previously emitted the platform-wide
APIError shape (stable code under "code", human under "error").
The agent surface uses the inverted shape (stable code under
"error", human under "message"), so adding action capabilities
to the manifest as-is would have forced agents to remember which
envelope each capability uses.

The slice refactors actions.go to emit the agent-stable envelope
across all 42 writeErrorResponse call sites. writeJSONError gains
a writeJSONErrorWithDetails sibling so the 13 calls that pass
field-level reasons (validation failures) preserve that
information under a new optional `details` field. The action
endpoints' JSON shape becomes:

  {"error": "<stable_code>", "message": "<human>",
   "details"?: {"<field>": "<reason>"}}

Frontend impact: zero. Verified that no frontend code consumes
the three action endpoints; the refactor is API-only.

Three new manifest entries (plan_action, decide_action,
execute_action) under a new "action" category, with their
declared error codes pinned per capability. Internal-failure 5xx
codes (audit-store outages, encode failures) are not declared
per capability; agents branch on 5xx generically.
TestContract_AgentSurfaceErrorCodesMatchManifestDeclarations now
audits actions.go alongside the existing two handler files, with
a documented internal-only allowlist for the 5xx codes.

The TestAgentSubstrate_ActionEndpointsEmitAgentStableEnvelope e2e
test exercises one error path through each endpoint via the actual
HTTP boundary, asserting the agent-stable envelope reaches the
wire and the legacy APIError fields (code, status_code, timestamp)
do NOT — drift back would mean the refactor regressed.

The TestContract_ActionDryRunOnlyExecutionErrorJSONSnapshot pin
is updated to match the new envelope shape; the manifest's
category allowlist gains "action".

api-contracts.md documents the new envelope (with details map),
the action governance loop's place in the substrate, the
ai:execute scope distinction from monitoring:write, and the
"manifest projection has a footnote" trade-off: bringing an
existing endpoint into the agent surface may require migrating
its error envelope, but the substrate keeps a single envelope
contract rather than carrying a translation wrapper layer.
agent-lifecycle.md and storage-recovery.md document the action
surface joining the agent paradigm and its zero-new-persistence
posture respectively. AGENT_SUBSTRATE.md's "what it does not do
yet" no longer lists the action surface; it now reflects the
real outstanding items (consumer feedback, an in-Pulse agent
integrations panel, a distribution path for pulse-mcp).

rcourtman bea3bbe5f6 Fix API token authentication and multi-tenancy logic
- Fix AuthContextMiddleware to use tenant-specific config for token validation

- Resolve data race in token LastUsedAt update

- Fix invalid org IDs returning 501/402 instead of 400

- Prevent unauthenticated organization directory creation (DoS protection)

rcourtman c4ca169e2b feat: add multi-tenant isolation foundation (disabled by default)
Implements multi-tenant infrastructure for organization-based data isolation.
Feature is gated behind PULSE_MULTI_TENANT_ENABLED env var and requires
Enterprise license - no impact on existing users.

Core components:
- TenantMiddleware: extracts org ID, validates access, 501/402 responses
- AuthorizationChecker: token/user access validation for organizations
- MultiTenantChecker: WebSocket upgrade gating with license check
- Per-tenant audit logging via LogAuditEventForTenant
- Organization model with membership support

Gating behavior:
- Feature flag disabled: 501 Not Implemented for non-default orgs
- Flag enabled, no license: 402 Payment Required
- Default org always works regardless of flag/license

Documentation added: docs/MULTI_TENANT.md

rcourtman 289d95374f feat: add multi-tenancy foundation (directory-per-tenant)
Implements Phase 1-2 of multi-tenancy support using a directory-per-tenant
strategy that preserves existing file-based persistence.

Key changes:
- Add MultiTenantPersistence manager for org-scoped config routing
- Add TenantMiddleware for X-Pulse-Org-ID header extraction and context propagation
- Add MultiTenantMonitor for per-tenant monitor lifecycle management
- Refactor handlers (ConfigHandlers, AlertHandlers, AIHandlers, etc.) to be
  context-aware with getConfig(ctx)/getMonitor(ctx) helpers
- Add Organization model for future tenant metadata
- Update server and router to wire multi-tenant components

All handlers maintain backward compatibility via legacy field fallbacks
for single-tenant deployments using the "default" org.
