* fix(mcp): bulk-update refuses an undeclared status/priority per item on stdio and remote too (BUG-3156, ToolSurfaceVersion 0.46)

Over WebMCP, pad_item bulk-update has refused (since 0.44) a status or
priority change on an item whose collection declares no such field; stdio
and remote went through the single-item PATCH, which accepted it with a
warning and stored an orphan field.

The item update door gains a general, additive strict opt-in,
`refuse_undeclared_fields`: with it, the write is refused with 400
validation_error for exactly the keys warnings.undeclared_fields would have
named, and nothing is written. Both bulk-update transports set it. A write
without it is unchanged; a server that predates it ignores it, which is the
old accept-and-warn.
