* feat(decision): log each evaluation's spend (TASK-3119)

One info line per evaluation that reached the provider: set, workspace,
item ref and id, questions, calls, input and output tokens, truncated,
outcome (stored|failed). Counts and identifiers only: no question, answer,
item text or error message. A failed call's tokens are counted too.

The dogfood box's /metrics is loopback-only and :7777 binds the LAN IP, so
the server log is where its spend is readable.

* test(decision): a liveness stop between split calls still logs the call made (TASK-3119)
