Every pad_item.action=history summary row, on both transports, gains an
omitempty actor_name: the display name of the user who made the change.
Rows said only created_by "user" or "agent", although the server joins
the name on every version read. Empty for rows with no user (recovery
writes, versions from before migration 103). ADDITIVE, on the v0.53 and
v0.28 grounds; no name, enum or param moved. The CLI's projection, which
stdio returns as it stands, moves into toItemVersionSummaries so it can be
tested. Each transport has a leg that fails without its copy.

Requested by the lead as its own PR before TASK-3321 U2, so the ChatGPT
item_history tool can say who changed an item again.
