Oliver Kieselbach 3a89319472 feat(telemetry): startup duration metrics for backend and MCP ...

StartupTelemetryService emits BackendStartupMs (process start ->
ApplicationStarted) and BackendTableInitMs as App Insights custom
metrics with version/commit/tableInitFullPass/instance dimensions;
TableStorageService exposes LastInitialization. MCP server writes one
{"type":"boot","readyMs"} stderr line per start next to the
tool_call lines so cold-start time is queryable as a trend.
