Claude deb28d1b49 fix(backend): shrink timeout grace buffer 12h→3h (48h cap → 51h grace) ...

A completion can only arrive <= the agent's absolute cap (it self-destructs and
sends nothing after), so the grace buffer needn't wait hours for stragglers — it
only has to span one 2h maintenance-sweep cycle + minor clock skew, and reconcile
heals any premature Incomplete. Buffer 12→3h, default grace 60→51h.
